Please call our office and have your insurance card ready. We will verify insurance eligibility before your appointment date.
Patients using their insurance will be informed of their benefits and co-pays before treatment. Co-pays are due when services are rendered.
Financing is available with Care Credit and Citibank.

If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
